Recently I've found Six Sigma Reports based on SIPOC model. I want to use them. But i can't figure the relationship between attributes of objects and cells of report. For example there is a report named FMEA which includes 'severe' column. Is this column manually filled on excel sheet or gets this data from an attribute?

Hi Alex,
the Six Sigma report FMEA is only a template. You have to enter the required data. Only the calculation of RPN works without having to enter data.
