I have been requested to edit the script of the Change Request Form, where can I find the script to amend?

Do you mean the standard form that opens when you click the button "submit change request"?
That's part of a workflow. You find that in the APG database- the workflow is called "Change request management (with e-mail integration)". In the underlying EPC process you can find the respective form. To edit it you can right-click on the element and select edit dialog.
I would only alter it, when you familiar with APG workflows.
