We are trying to add a customized report to Publisher, it uploads the document showing no name or description (below screenshot)

While evaluating a report in Publisher, it shows no evaluation name but a blank section to select as below screenshot. After running, it gives the error Evaluation failed to complete.

The properties of the customized report are as below. It’s contexts are Models and Objects (all of them selected) and outputs a word or pdf file. In General Release the checked boxes are Open dialogs, Available to Users and Available in ARIS Connect. What might be causing this problem to occur?

Hi!
How does it look on the server, if you check the webapps/businesspublisher/reports/ , do you find both the .script,.loc and .info files? And is there a name set in the .loc file?
